SR-22 stands for "Safety Responsibility" Form #22. An SR-22 is usually required by the DMV after your license has been suspended for reasons such as:

    DWI
    Accident without insurance
    Driving on a suspended license
    Multiple driving offenses
    Failure to pay fines

An SR-22 can be purchased as a stand alone policy on your drivers license or in addition to your car insurance policy.

How it works:
After writing a policy with an SR-22, the insurance company will file the SR-22 form with the DMV for the duration of your policy term. In the event your policy is canceled for any reason, a notification is then sent to DMV who in turn suspends your license until a new policy is written and a new SR-22 is filed with DMV.

Most common time requirements for an SR-22 is one year. DWI offenses carry a 3 year SR-22 requirement. SR-22 premiums are usually much higher than standard car insurance rates because the driver, in most cases, has violations on their record.
